Description:
A dynamic and enthusiastic associate practitioner is required to work in a busy award winning mental health physical health and wellbeing service. This post will be for an agile worker supporting existing physical health and wellbeing services, but also working alongside additional teams such as older peoples, EIP and IOT teams.
The successful applicant will have a good knowledge of physical health in mental health and be able to demonstrate a passion for this area. The post holder will also need the ability to work agilely and posses skills in venepuncture and ECG recording. An understanding of community physical health services and voluntary sector will also be an advantage.

To provide physical health monitoring and interventions to people requiring annual health checks or when initiating new medication, within the busy CMHT wellbeing clinics. There will be an opportunity to work in the outpatient setting monitoring health within the busy Clozaril clinic. This will be under discussion with team manager.
To deliver quality care and support which enables clients to be supported
throughout their care pathway whilst promoting health and wellbeing.
To work alongside both registered and unregistered staff running clinics,
supporting colleagues and facilitating groups.
To communicate effectively at all times within the team, with other health
care professionals and with the client and their family.
