Description:
Please see uploaded job description and person spec. - Providing meal support, psychoeducation and emotional support to young people and their families who are effected by an eating disorder within Bristol, South Glos and North Somerset. - Providing support in young people’s homes to prevent hospital admissions where possible. - Working alongside colleagues within the ED Home Treatment Team, and wider colleagues from Acute trusts and CAMHS.
- Working autonomously at times, with indirect supervision from registered staff within the team. - Keeping clear and accurate records of appointments including; engagement, Safeguarding concerns, food and fluid intake & interpersonal communication with a family and care team. - Supporting young people and families at home to manage periods of distress and upset. - Supporting young people and families to develop positive ways of managing and coping, - Providing support in conjunction and as an extension of Family Based Treatment for an ED which will be provided by the Locality CAMHS Team.
- Working in a person centred way with young people who will have different diagnoses, co-morbidities and needs within an Eating Disorder support framework. - Assessing and supporting young people with physical health symptoms, with clear care plans and escalation plans. This role includes working alongside our colleagues from acute settings, you will be part of a multi-disciplinary team, which at times supports young people who are fed by nasal gastric tubes in their home for short periods of time.
