Description:

A great opportunity has arisen for a Band 4 Recovery Co-ordinator to join the West Wiltshire Community Mental Health Team based in Warminster. This is a full time position working Monday to Friday 9-5pm.
You would be joining a friendly, sociable, cohesive, and pro-active team and you would be well supported by those you work with.
The role is focussed on supporting people with severe and enduring mental health needs, and their carers (friends/relatives/carers) and their supporters in the community, enabling and assisting them to meet daily health, social care and well being needs, in line with personal recovery goals, and facilitating engagement with mainstream services.
In West Wiltshire CMHT we have two pathways; the non psychotic pathway which is for service users with personality disorder, complex emotional needs, PTSD, depression and or anxiety. The psychotic pathway which is for service users with a psychotic illness or a mood disorder. We are currently recruiting a Band 4 position on each pathway.
The post holder will contribute to the ongoing assessment, planning delivery and review of activities and interventions against identified health, social care and well being needs, acting as care/recovery coordinator for an allocated group of service users. The role will also require undertaking and delivering defined activities and interventions for a wider group of identified service users, in accordance with the agreed personal recovery plan. This maybe either on a one to one basis, or as part of a group activity.

1. To undertake the role of care/recovery coordinator for an allocated caseload of service users organising and participating in the ongoing assessment, planning, delivering and review of care to meet identified physical and mental health social care and well being needs. This will include:
Explaining the role of care coordinator.
Organising and participating in defined assessment methods/activities.
Arranging CPA meetings in line with trust policy.
Developing and reviewing appropriate care plans identifying and organising other resources as appropriate.
Completing relevant paper and electronic documentation in line with the care coordinator role.
2. Planning and undertaking defined interventions and treatments to enable the individual to meet and develop their ongoing daily living skills, to increase confidence and independence in line with agreed personal recovery plans. This might include;
Using the recover Star as a framework for planning care.
Preparing for and delivering defined self-help groups.
Therapeutic activities to improve and individuals coping skills e.g. anger,
anxiety, stress management, assertiveness or relaxation.
Therapeutic activities to improve an individual’s physical health such as
exercise programmes, diet or nutritional planning.
3. Planning and undertaking activities to enable the individual to meet and develop their daily living skills, to increase confidence and independence in line with agreed personal recovery plans. This might include helping individuals to:
access information on appropriate and safe housing;
move into new accommodation or respite care;
complete forms or letter e.g. housing benefits;
Contact employment advisors or education facilities.
