Job Introduction
We have an exciting opportunity for a Band 4 Qualified Nurse Associate to come and join our Malmesbury Community Team at Wiltshire Health and Care. Community Teams within Wiltshire Health & Care provide high quality, complex care in the community setting, which includes caring for some very vulnerable people, often in challenging environments. Wiltshire Health & Care recognises that we need to deliver care closer to home, with the resources available, and be able to meet the needs of the local population. You will be expected to work closely with general practices, adult social care, local hospitals and other statutory and non-statutory services to meet the needs of the community, so excellent communication skills are essential for this role.
Experience in front line NHS community patient care is not essential as we are keen to attract new graduates as well as experienced Nurse Associates, but evidence of up-to-date clinical professional development is necessary. The role is satisfying and rewarding, and we are looking for enthusiastic, motivated, and hardworking individuals who can work flexibly and efficiently. We are looking for a friendly, caring, motivated person to join our active, professional, and dynamic team.
The aim of the role is to care for people in their own home holistically, so they are empowered to control their health and recovery journey. You will also be expected to support patients with long term conditions and end-of-life care.
Malmesbury Community Team is a busy team caring for patients registered with a Malmesbury GP, you will have the opportunity to work alongside other members of the Multidisciplinary Team such as Occupational Therapists, Physiotherapists, Assistant Practitioners, and other Specialist’s. Our Shift patterns are 7-3, 8.30-4.30, 10-6 and 2-10pm and you would work approximately every 4th weekend however flexible working may be available.
We provide a huge amount of clinical skills training and potential further university courses when you have achieved all the skills necessary for your current role.

- To work in care teams with other professionals, providing practical help to registered practitioners by delivering delegated aspects of nursing care to vulnerable and often highly dependent patients with a range of illnesses.
- To use sound clinical reasoning skills when delivering care and therapy interventions in line with goals agreed with the individual and set by a registered health professional, this would only be expected following appropriate training or when competency reached.
- To be able to accurately and effectively report on care undertaken and document changes in condition, maintain accurate and contemporaneous records, in hand-written or electronic format and ensure that there is feedback to the appropriate or responsible registered healthcare professional.
- To establish good therapeutic relationships with individuals and be able to communicate effectively in order to overcome barriers to understanding when treating individuals who may have physical or mental health issues or learning difficulties that affect their ability to communicate, understand or reason.
- To assist in the maintenance, cleaning and storage of equipment.
- To attend team meetings and appropriate education and training to ensure continuing professional development.
- To actively participate in setting personal objectives through appraisal and clinical supervision.
- To participate in active rotation to support the key objectives of course delivery
- To follow Medicines management and administration standard operating procedures for the organization and follow policy for safe and effective administration and optimization of medicines. This should be in accordance with NMC standards of proficiency for Nursing Associates
The Ideal Candidate
- Registered as a Nurse Associate with the Nursing and Midwifery Council (NMC) • Car driver
- Excellent communication skills.
- Able to work independently, using own initiative and knowing when to seek advice.
