Description:

  1. Core Duties and Responsibilities (Key Result Areas) To offer a comprehensive service to the resident population, providing appropriate assessments, following National Framework Guidelines for Continuing Healthcare. Undertaking Continuing Healthcare/Funded Nursing Care/ Section 117 Aftercare assessments and reviews for patients across all aspects of care. This necessitates a high level of assessment skills and requiring tact, diplomacy and the ability to liaise effectively with colleagues, health care professionals, patients’ carers and family members.
    The timely review of all Continuing Healthcare/Funded Nursing Care/Section 117 patients functioning as a central point for referral, assessment and support. Supporting the local resolution process in the case of complaints/appeals and providing information on a timely basis during the assessment process. Liaise with multi-disciplinary members in secondary care, primary care, nursing homes etc. to facilitate the continuing healthcare process.
    Developing and maintaining effective and appropriate communication networks with health and social care professionals. Interpreting national and regional guidance relating to the implementation of the National Framework and subsequent developments within all the Continuing Healthcare Services. Exercising professional judgement as necessary when communicating information on behalf of the Continuing Healthcare team and application of the associated criteria. Working with relevant health professionals, Continuing Healthcare colleagues, commissioning colleagues and other key stakeholders to ensure delivery of national targets and key performance indicators.
    Supporting formal and non-formal education programs to Community and hospital staff including nursing/residential homes, GPs, INT and therapy staff relating to the assessment and management of Continuing Healthcare patient groups in line with the latest research, evidence and DOH/NICE guidance. Facilitating timely Fast Track reviews, following the national guidance for patients Fast Tracked through the Continuing Healthcare system. Facilitating smooth patient flow across all aspects of the patients journey including liaison with primary care, secondary care, community venues and specialist services. To support the development across the organisation in the Integrated personal commissioning agenda and to support the pathway delivery for Personal Health Budgets within NHS Continuing Healthcare Ensure data systems are kept up-to-date and maintained in line with standard operating policy and procedures.
    To comply with the Data Protection Act 1998 and Code of Practice on confidentiality and data protection To participate in ongoing professional development and clinical supervision in line with NMC guidelines To ensure compliance with all statutory and mandatory training To ensure revalidation of professional registration is maintained. To provide cover for other Nurse Assessors as appropriate To ensure that communication remains professional and appropriate at all times. To support the Continuing Healthcare department in working towards the Directorates goals and objectives Required to maintain constructive relationships with a broad range of internal and external stakeholders. To work closely with all care providers involved in delivering the service.
    Provide coordination of and participate in relevant meetings, reporting attendance and providing information advice and support where requested. Ensure that data collected is reported as appropriate and monitor the processing of data and information. Provides relevant and timely specialist advice and guidance on functional and information matters relating to CHC and associated services. The post holder will be required to deliver specialist associated services pertaining to their skillset.
    (Mental Health, Learning Disabilities, Adults) To provide accurate and legible recording of information. To assist/support individuals and representatives in relation to NHS Continuing Healthcare process. Operate within and provide enhancements to current management information, reporting to enhance decision making processes. Deliver against objectives, performance targets, achieving quality outcomes, prioritising own workload and working to tight deadlines.
    Support training and induction of new staff. Undertake research and development as directed. The post holder will carry out any other duties as may reasonably be required by their line manager.     Supplementary Duties & Responsibilities Mobility Employees may be required to work at any of the other sites within the organisation subject to consultation. The organisation is in a period of rapid change due to developments and rationalisation of services. This will lead to modification of structures and job descriptions.
