Description:
Maintain own patient case notes and provide written documentation in medical and/or nursing notes. Attend and participate the weekly Lung MDT meeting, contribute to the discussion, act as an advocate. Work with Lead Lung CNS to ensure decisions are effectively coordinated and communicated to other health professionals or to the patient. Provide up-to-date relevant written information to patients and families regarding lung cancer, its diagnosis, treatment, and long-term care.
Professional leadership 1.Provide a credible source of specialist clinical knowledge and advice, taking an active role in self-development, reviewing own performance, identifying own needs, and taking active measures to meet those needs. 2.Provide on-going education for patients and their families about all aspects of their illness/treatment. 3.Participate in the formal and informal teaching within the hospital to a range of health care professionals and contribute to educational and training programmes as requested / appropriate. 4.Participate, where appropriate, in the education of patients and the general public.
5.Maintain an awareness and understanding of relevant, up to date research studies within the clinical field. EDUCATION AND TRAINING The post holder will /work towards: 1.Participate / facilitate study days relating to Lung Cancer. 2.Act as an educational resource on lung cancer care for healthcare professionals both in hospital and the community. 3.Provide education to patients and carers in relation to lung cancer.
4.Incorporate teaching and learning principles into professional practice. 5.Act as a role model or mentor when appropriate. RESEARCH, DEVELOPMENT AND AUDIT The post holder will: 1.Be involved in local/national research proposals to develop lung cancer services nationally. 2.Disseminate research findings locally and, if required, at national level to produce an evidence base for lung cancer nursing on behalf of the Trust and the Cancer Network.
3.To be aware of and disseminate recent advances in lung cancer. 4.Participate in the collection of statistical information to inform the operation activities of the lung cancer service, specifically in relation to the National Lung cancer Audit (NLCA), working with Cancer Data Manager as required. COMMUNICATION The post holder will: 1.Communicate sensitive and distressing information to patients and relatives daily. This may include information concerning diagnosis, prognosis, recurrence, or impending death.
2.Utilise a wide range of advanced communication skills to address the complex needs of patients and their families. 3.Assess barriers to understanding during complex interactions. 4.Communicate the special needs of patients and families to those responsible for their direct care. SKILLS The post holder will / work towards: 1.Use advanced skills to make patient and family needs assessments and deliver expert care.
2.Maintain up-to-date knowledge of national developments within lung cancer. 3.Maintain knowledge base to provide clinical palliative care. 4.Provide a non-pharmaceutical intervention to help patients with problems of breathlessness. 5.Provide an intervention aimed at reducing anxiety and depression in patients with lung cancer and helping them to cope with the illness.
Professional Development 1.Contribute to the development of team objectives. 2.To be aware of the impact of service changes across the organisation 3.Contribute to the monitoring and control of expenditure and the use of resources. 4.Comply with Health & Safety and Infection Control policies and ensure that staff, patients, and visitors are appropriately informed and advised. Maintain a clean and safe environment.
Ensure equipment used and techniques employed by staff comply with the Directorate Health & Safety policies. EFFORT AND ENVIRONMENT The post holder will: 1.Participate in providing a seamless service that reaches across hospital and community. 2.Have the ability and skills required for situations that are stressful and have barriers to communication. 3.Participate in any physical care required by the patient at the time of visit and may include the administration of appropriate medicines to relieve distressing symptoms and deal with bodily fluids.
4.Care for terminally ill patients, supporting their families and health professionals caring for them. 5. Be able to concentrate for long periods for report writing, research activities, audit and patient contact.
