Description:
We are looking for an experienced mental health clinician to manage a caseload of people from Wiltshire who are either placed in an out of area specialist placement or a forensic setting. The role is to oversee the pathway for the duration of an individuals treatment whilst placed out of county.
This includes care co-ordination, review of placement, discharge planning and repatriation to Wiltshire. Experience of advocating for patients and liaising with families is key.
The role involves traveling to sites some distance from base.
The ability to develop relationships with a wide range of organisations, providers and the ICB is essential.
Undertaking assessments of need and risk and formulating and/or contributing to MDT/multi agency care planning processes. Supporting community mental health services in compiling and presenting requests for specialist placements funded via ICB or Local Authority funding panels. Providing and presenting statutory reports including MHA Tribunal reports. Undertaking reviews of care and producing and presenting documentation to funding panels.
To deliver teaching/training and supervision as appropriate to the role and to offer consultation to other colleagues in relation to out of county community providers and specialist placements . To represent AWP, Wiltshire at local Multi Agency Puplic Protection Arrangements, MAPPA.
The post requires the post-holder to hold their own caseload. To work collaboratively with other agencies and work collaboratively across geographic boundaries.

To work as an independent practitioner with service users referred to the Specialised and Secure Services at Fromeside, with a balanced caseload of individual assessment/treatment, group, consultation/liaison cases.
To be involved in planning care pathways for service users in the independent sector of medium security, including assessing the suitability of placements and supporting teams to manage care pathways.
To become involved in direct patient care in preparation for discharge, conditional discharge and patient transfers appropriately during patient rehab.
To communicate, for prolonged periods of time highly complex and sensitive information to distressed and/or aggressive service-users including assessments, formulations and treatment options.
To provide peer clinical supervision and consultation to other staff in the Trust and to colleagues outside the Trust where appropriate.
To support the full implementation (including care coordination) of the Integrated Care Programme Approach in Specialised and Secure Services.
To participate in regular clinical and caseload supervision arrangements provided by Specialised and Secure Services in accordance with good practice guidelines.
To maintain accurate records and to monitor clinical workload using agreed systems
