Description:

The Brent Health Matters Clinical Team, are looking to recruit a team of enthusiastic and forward thinking GP’s to join our dedicated and friendly team, to help us deliver an integrated approach to the care , to vulnerable patients that do not normally engage with health services that have been referred by local GP’s or identified through staff interventions by BHM team .
The Brent Health Matters Clinical Service delivers bespoke, holistic, health and social care assessments, to the identified patient cohorts and require GPs to provide their clinical knowledge, expertise and support, to ensure the provision of its high quality, effective and safe patient care.
The aim of the Brent Health Matters programme is to reduce the variations in health inequalities, which in turn will support our communities to better manage long term conditions.
This will include seeing these patients in a clinic based setting, at their home or other setting in the community.
Respond to medical problems presented by patient’s or the clinical team, including history taking, examination, supporting diagnosis and requests for referrals, where appropriate, to the registered GP
To provide clinical oversight to the team including reviews of the services Quantitative and Qualitative KPI’s.
To assist and support relationship building with key stakeholders that may use or refer into the service
To provide clinical GP support, feedback and strategies at agreed BHM community events.
Conduct clinical reviews with patients, in their homes or at a community / clinic setting
Provide a weekly board round to discuss case managed patients that may require medical interventions.
Liaise with the registered GP’s to identify high risk patients following weekly ward rounds, for appropriate follow-up at the patient’s practice or within the community setting
To provide professional support to the services clinical pharmacists, on medication care plans and communicate these with other health and social care professionals e.g. PCN or Community Pharmacists for Medicines Use Reviews (MURs) and New Medicines Service (NMS) in the absence of the pharmacist.
Deliver in house teaching / training sessions to the clinical team, in order to improve the delivered clinical outcomes of the service.
To plan, review and discuss the available QOF data, for long term conditions as highlighted for particular Brent GP practice performances.
Develop / provide and ensure innovative planning and clinical service delivery to future patient cohorts and collaborative discussions with those Brent GP’s as required
All staff must comply with the Central London Community Healthcare (CLCH) NHS Trust Staff Code of Conduct and demonstrate the Trust’s Values and Behaviours. Senior Managers must also comply with the NHS Code of Conduct for Managers, based on the Nolan principles of public accountability. All staff employed in recognised professions are required to ensure they work to the professional standards and/or Codes of Practice set out for their professional group. In addition, staff are required to demonstrate the Customer Care Standards of the organisation.
The delivery of highly effective medical care to the identified population
The provision of services commensurate with the GMS contract
Ensure the effective management of long term conditions
Contribute to the successful implementation of continuous improvement and quality initiatives within the clinical service.
Commit to self learning and instill an ethos of continuing professional development across the clinical team.
Support the clinical team in achieving its strategic and operational aims, making recommendations to enhance income and reduce expenditure.
Encourage collaborative working, liaising with all staff regularly, promoting a culture of continuous improvement at all times.
Participate in service audits
Assist in the completion of QOF and other local enhanced service targets
Participate in local initiatives to enhance service delivery and patient care.
Participate in the review of significant and near miss events applying a structured approach (Root Cause Analysis)
Take responsibility for abnormal and urgent patient findings and results highlighted by the team, to manage as appropriate.
Ensure that the appropriate pathways are in place, that will enable the clinical team to escalate any concerns to the registered or event GP, report abnormal patient’s result
