The postholder will work within the multidisciplinary Barnet Respiratory COPD team to deliver Pulmonary Rehabilitation (PR) to adult patients with chronic respiratory conditions and heart failure across the London borough of Barnet. There is currently a drive to tackle health inequality within the boundaries of pulmonary rehabilitation in Barnet located in North Central London as directed by NHS England. As a band 6 specialist cardiorespiratory physiotherapist, you will be expected to support the PR Health inequality quality improvement (QI) project, led by the PR clinical Lead, and work day-to-day in line with the aim of including patient groups who qualify to take part in PR as per criteria but are not currently included in our PR sessions due to issues underpinning health inequality and access.

• To use specialist respiratory knowledge and skills in proactively selection of suitable COPD patients for pulmonary rehabilitation utilising NICE and British Thoracic Society (BTS) guidelines, Association of Chartered Physiotherapist in Respiratory Care (ACPRC) guidelines, British Association of Cardiac Rehabilitation (BACR) guidelines and Association of Chartered Physiotherapists in Cardiac Rehabilitation (ACPICR) guidelines.
• To work in line with the PR health inequality Q I project with a clear objective of case findings of patients at risk of experiencing health inequality
• To undertake pre pulmonary rehabilitation clinics to ensure appropriate patients are selected and clinical risk is minimised.
• To undertake and supervise others in running evidence based pulmonary rehabilitation and maintenance programme, ensuring that patient outcomes can be demonstrated.
• To undertake comprehensive assessments of respiratory and heart failure patients with co-morbidities, including physical examination, and to develop treatment plans for patients using investigative and analytical skills .
• To assess, develop and implement specialist care programmes, including pulmonary rehabilitation with evidence-based exercise prescription and progression, home oxygen, breathlessness, airway clearance, psychological interventions, and medication reviews, adhering to evidence based guidelines.

• To communicate effectively with relatives, carers, patients, and other professionals to promote understanding of the aims of the specialist respiratory service, to ensure understanding of condition and to ensure a consistent approach to patient care.
• To communicate effectively and work collaboratively with medical, nursing, and allied health professional and other colleagues to ensure delivery of a coordinated multi-disciplinary service.
• To provide specialist advice on the management of cardiorespiratory patients to colleagues working in other clinical areas.
• Communicate and report PR Q I health inequality project to the clinical lead regularly and may present data to the trust innovation team.
• To be contactable throughout the working day via the mobile phone system with the flexibility to change planned activity sometimes in order to respond to arising situations.
