Description:

  • To deliver a comprehensive medicines management service to HMP Bedford patients and the multidisciplinary treatment team responsible for their care.
  • To provide expect advice on pharmaceutical matters relating to primary care, substance misuse and mental health services, this will include assistance with risk management and ensuring compliance with medicines legislation and local medicines management policies.
  • To provide input into policy and procedures affecting the use of pharmaceuticals at both local and trust wide levels under the direction of the advanced specialist pharmacist.
  • To liaise with HMP Bedford pharmacy staff to support an effective dispensing and supply service.
  • To participate in clinical audits and research associated with medicines and pharmacy practice at HMP Bedford.
  • To deputise for the Advanced Specialist Pharmacist in his or her absence.
  • Ensure that all prescriptions and Medication Administration Records are clinically appropriate and meet all necessary legal requirements
  • Ensure safe and appropriate use of medicines in order to maximise benefit and minimise risk to patients. This includes regular reviews of prescriptions, identification of significant drug interactions, advice for clinical monitoring, compliance with legal aspects (Medicines Act, Mental Health Act) of medicines supply and compliance with the Trust Formulary
  • Assist the advanced specialist pharmacist on the implementation of medicine related NICE publications, ensuring that recommendations form CNWL NHS Foundation Trust Medicines Management Group are considered and that systems for auditing and reporting on uptake are established.
  • Be responsible for providing clinical pharmacy input to the multidisciplinary team caring for patients
  • Promote cost effective use of Medicines and other equipment at HMP Bedford.
  • Promote the use of a preferred prescribing List (Formulary), liaising with appropriate healthcare professionals to ensure the cost-effective use of clinically effective prescribed drugs.
  • Ensure subordinate pharmacy staff providing a service to HMP Bedford have adequate competencies to deliver such service.
  • Be responsible for providing a clinical pharmacy service to groups of patients:
  • Review prescriptions for accuracy and legality and identify actual or potential problems
  • Ensure all medications are recorded in the electronic prescribing system (SystmOne)
  • Ensure the effective supply of medicines suitable for use (IP/NIP) and appropriately labelled for discharge or court if applicable.
  • Ensure the effective outcomes of treatment with medicines
  • Monitor patients for potential and actual adverse effect of their medicines
  • Ensure all pharmacy interventions are recorded
  • Take steps to ensure patients understand the purpose of their medicines and deal with any practical issues that may prevent the optimal use of their medicines
  • Support the planning of patients release with respect to medication, including the transcription of requests for dispensing
  • Demonstrate professional accountability to patients

  • Manage and make appropriate referrals to other members of the multidisciplinary team and within the pharmacy department
  • Be able to critically appraise the literature and give accurate interpretation in order to improve use of medicines
  • Reduce the risk associated with medicines use by contributing to the safe medication practice agenda. This will include identification and reporting of medicines related errors
  • Develop and demonstrate expert clinical knowledge and motivate and inspire others
  • Act as a clinical role model and demonstrate the ability to provide safe, clinically effective and cost-efficient use of medicines for junior pharmacy staff
  • Undertake clinical visits to designated House units
  • Provide pharmaceutical advice to health and associated staff as required.
  • Advice provided is accurate, evaluated, impartial and up-to-date to ensure the safe, clinically effective and cost-efficient use of medicines in patients.
  • Monitor the usage of Controlled Drugs in each clinical areas.
  • Maintain the medicines reconciliation provision.
