Overview:
Novant Health is seeking a Behavioral Health Care Specialist for our therapy team on the Inpatient Unit at Rowan Medical Center. The BH Care Specialist works among an interdisciplinary team of clinicians and psychiatric providers to assess patients’ needs, collaborate with the patient to identify a plan for aftercare and safe living arrangements post-discharge, and facilitates execution of this plan.
The care specialist works to ensure safety, best practice and high-quality standards of care maintained throughout the discharge planning process and establishes care connections with providers when patients leave the hospital. They collaborate with providers and all members of the interdisciplinary treatment team to assess, plan, implement and evaluate proactive complex and safe discharge plans with patient involvement. The care specialist serves as a resource for discharge planning and care coordination, requiring familiarity with local community resources and various levels of Behavioral Health Care. They must demonstrate the knowledge and skills necessary to provide services appropriate to a patient population comprised of various ages (18+), levels of acuity, and mental health and/or substance use diagnoses. Previous experience in the hospital setting or with crisis response would be highly valuable in this role.

Responsibilities:
- Participating in an interdisciplinary treatment team to identify patient needs, potential barriers, and appropriate next steps for care.
- Ensuring an individualized safe discharge plan is in place for each patient including arrangements for aftercare treatment, identification of living arrangements, and plans for transportation post-discharge.
- Understanding patient’s appropriateness for various levels of Behavioral Health Aftercare and having the ability to discuss these options for treatment with assigned patients (ie: Outpatient therapy, IOP/PHP, Residential Treatment, State Hospitalization, Long Term Facility Placement) and schedule hospital follow-up appointment to ensure continuity of care.
- Involvement in petitioning for guardianship when indicated.
- Arranging patient transport.
- Providing information and education to patients regarding available community resources in their area based on each patient’s resources and identified barriers.
- Conducting groups related to discharge planning, aftercare education, relapse prevention, etc.
- Completing safety planning with patients upon discharge.
- Participates on the Psychiatric Rapid Response Team to assist in de-escalation of patients in other areas of the hospital during behavioral health crises.
- Timely and accurate documentation of all patient interaction and discharge planning
