JOB DESCRIPTION:

• The Behavioral Health Consultant/Counselor works as a core member of the collaborative care team, including the patient’s medical provider, psychiatric consultant, and the larger primary care/medical team.
• The behavioral health consultant provides counseling, diagnostic and treatment planning, supports medication management and facilitates communication and planning with the patient and team members.
• Position offers new grads the opportunity for counseling experience and clinical supervision for 3000 hours.

QUALIFICATIONS:

• Master’s degree in Social Work, Psychology, Counseling, or related field required.
• State of Wisconsin license or in-training license such as Professional Counselor Training License or Advanced Practice Social Worker required; may also be certified Substance Abuse Counselor.
• Previous experience in pediatric, family, and adult counseling required.
