QUALIFICATIONS:
Required:
- High School graduate or equivalent.
- Must have reliable transportation
- Must have skills with the ability to email, navigate Microsoft Work, and other software systems.
Preferred:
- BA in Social Work preferred
- Prior experience as medical office assistant, medical assistant, or medical access/call center a plus
- Bilingual a plus

Responsibilities:
- Assist patients eliminating barriers to kept appointments such as, arranging for transportation and/or transporting the patient via company vehicle
- Conduct outreach to patient and community partners as needed
- Ensure effective communication with the entire team
- Ensure that assessments and other documentation required to support NJ-QIP program requirements are completed and follow up to ensure all data is collected in a timely manner
- Foster relationships with key stakeholders
- Gather patient information to complete required documentation
- Initiate post discharge follow-up for mental health services and support adherence to the appointment and communicate the discharge plan with the interdisciplinary team
- Meet the patient in the Emergency Department, PESS/Crisis units, Inpatient Units and in the community to engage the patient and navigate to the proper follow-up location
- Partner with multidisciplinary team during the development of relapse prevention plans
- Provide motivational interviewing interventions and provide timely feedback regarding the patient s care, treatment recommendations and progress via documentation in the client s record and verbal feedback.
