To help fulfill its mission, LOFT is hiring a Behavioural Support Specialist.

JOB SUMMARY

As a Behavioural Support Specialist, you will be a member of Behavioural Support Teams that provides direct care, education and support to seniors who are demonstrating responsive behaviours that may be associated with various types of dementia, delirium, mental health, addictions or other neurological conditions currently within acute care or post-acute in the community. The Behavioural Support Specialist will provide direct service to support the client, family members and/or care-givers, and other members of the care team to mitigate escalating responsive behaviours. In doing so, you will collaborate with the other members of the care team on the development and delivery of effective Behavioural Support Plans comprised of behavioural management approaches, psychosocial supports and education to referred individuals and their caregivers. You will also liaise with community agencies, other practitioners and key partners to coordinate care for clients. You will coordinate transitional support to ensure a successful and sustainable transition to the next care destination. In addition, you will provide education and support to the care team to ensure current knowledge, trends and ethical guidelines in behavioural support.
Staff will be required to work in clients’ homes and may therefore be exposed to second-hand smoke. There may be occasional evening and weekend hours and travel throughout the area will be required using own vehicle.

Responsibilities:

  • Provide clinical assessment and treatment recommendations through a comprehensive behavioural approach. Complete Behavioural Support Assessment and provide timely and accurate record keeping and reports, as required;
  • In collaboration with the care team, patient/client and others, develop and implement a Behavioural Support Plan tailored to the patient/client’s needs and issues designed to improve quality of life and assist in the mitigation of responsive behaviours;
  • Design and ensure implementation and monitoring of skill promotion and behaviour intervention programs for identified patients/clients;
  • Provide risk management and urgent care planning for patients/clients;
  • Design and implement all Behavioural Support Plans to ensure adherence to ethical guidelines in behaviour management;
  • Review and edit team members Behavioural Support Plans to ensure adherence to ethical guidelines in behaviour management;
  • Coordinate transitional support to ensure a successful and sustainable transition to the next care destination;
  • Act as a liaison and client advocate among those involved with the client’s care including family, other care-givers, medical and other multi-disciplinary providers;
  • Provide support, on-going training and consultation to colleagues, family members and/or other care partners to assist in mitigating escalating responsive behaviours;
  • Maintain up-to-date knowledge of current and developing behavioural support models and trends and disseminate this information to the multi-disciplinary team;
  • Develop respectful and trusting relationships with clients using both Person-and-Family Centred Care and Recovery/ Psychosocial Rehabilitation principles;
  • Maintain strict confidentiality and follow all other required policies and procedures of both LOFT Community Services and the partner organization;
  • Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization and the Behavioural Supports Ontario Project;
  • Effectively participate as a member of the interdisciplinary team; attend staff/team meetings, client care conferences, other multi-disciplinary meetings, committee meetings etc. as required;
  • Attend and actively participate in all required training;
  • Collaboratively participates in ongoing supervision with the Program Manager;
  • Liaise, collaborate and coordinate with other professionals, service providers and partners within the Behavioural Support Services;
  • It is an expectation of the position that some of the work will be conducted in hospitals and compliance with their occupational health polices and protocols will be required (associated costs will be reimbursed)
  • Complete other duties, from time to time, as assigned by the Program Manager.
