Description:
DESCRIPTION:
Provides integrative behavioral health (BH) services, clinical therapies, and an integrative partnership within the Community Health Center (CHC) under the supervision of the BH Director. This position has BH consultation and integration, and clinical therapies as its primary focus. This includes the implementation of BH services within the assigned CHC, timely and accurate patient documentation to meet regulatory compliance per FQHC requirements, contribution toward quality improvement/quality assurance activities BH services, and reporting data as it pertains to programs. The Licensed Clinical Social Worker or Licensed Mental Health Practitioner is responsible for maintaining licensure requirements. Demonstrate professionalism and teamwork with all health care providers to build collaborative relationships to benefit the patients served. Performs duties and responsibilities in a manner consistent with our mission, vision, and values.
1. Provides BH consultation and clinical therapies.
2. Provides BH resources development and partners with community organizations to provide continuum of care for Bluestem Health patient centered care.
3. Provides substance abuse services within scope of practice.
4. Participates in behavioral health staff meetings in partnership with medical providers.
5. Serves as a resource and positive, proactive leader within assigned CHC.
6. Complies with and provides input into behavioral health policies and procedures.
7. Stays current with behavioral health care trends and information that impacts the behavioral health practice.
8. Assists in maintaining an efficient, safe, and secure care environment for all patients and employees.
9. Provides input to BH Director to analyze the workflow and make recommendations and implements actions, as needed.
10. Contributes and completes responsibilities necessary for the quality assurance process.
11. Responsible for completing orientation, monthly training, and performance related activities.
12. Participates in BH strategic planning, as requested.
13. Provide input re: training needs, resources, and behavioral health materials for budget consideration by BH Director and Chief Financial Officer.
14. Engage in behaviors that optimize program efficiency, assuring quality and a positive customer service environment for integrative primary health care.
15. Other duties as assigned
REQUIREMENTS:
- Master’s in Social Work or Counseling from accredited educational institution.
- Ability to communicate & partner within a multidisciplinary environment.
- Demonstrates self-motivation and ability to work independently
- Experience working with people of different cultural backgrounds
- Ability to exercise sound judgment in appraising situations and making decisions
- Demonstrates ability to resolve interpersonal and professional conflicts appropriately
- Experience in HIPAA regulations
- Demonstrates critical thinking and problem-solving skills
- Excellent written and oral skills
