Job Summary:
Under routine supervision, collects data from birth certificate worksheets and electronically submits to Vital Records. The Birth Registrar ensures that all reports (typed and submitted) are accurate based upon information obtained from worksheets and verified with the patient.

Essential Functions & Responsibilities:

  • Types birth certificates using VDH software for electronic birth certificates.
  • Prints out hard copy for review and signature by the mother.
  • Places a copy on the Mother’s chart.
  • Delivers birth certificates to HIM for management signature; sends signed birth certificates to the Department of Vital Records.
  • Maintains paper backup of information in appropriate system.
  • Explains process of paternity papers to unwed mothers.
  • Assists parents as needed in completing paternity papers.
  • Reports all paternity documents weekly to the Virginia Paternity Establishment Program.
  • Identifies when fetal death forms are needed.
  • Completes demographic information on forms and forwards to Health Information Management.
  • Sends finalized forms to the Virginia Department of Health, Division of Health Statistics.
  • Sends monthly report of all fetal deaths to Fredericksburg Health Department.
  • Performs other duties as assigned.

Qualifications:

  • High school diploma required.
  • Six months of relevant experience is preferred.
  • Required to obtain Notary Public access within 60 days of hire or placement.
  • Performs clerical duties as requested. Transcribes physicians’ order to appropriate documents.
  • Performs selected patient registration activities.
  • Coordinates scheduling of patient tests, diagnostic procedures, and transports.
  • Inputs patient charges timely; clarifies accuracy of the level of care issued at the point of entry and follows up as appropriate.
  • Reviews patient chart to ensure all procedural documentation and consent forms are present.
  • Posts patient diagnostic results in patient’s chart.
  • Facilitates communication between diagnostic/testing area, patient care area and families.
  • Demonstrates effective communication, customer service, confidentiality, and team building skills.
  • Answers patient call light system.
  • Communicates patient and family requests to appropriate personnel immediately after receipt of request.
  • Coordinates equipment/supply needs; notifies appropriate departments regarding equipment malfunction in collaboration with Unit Manager.
  • Assists the Unit Manager with regulatory readiness, QI Activities, emergency procedures and with hospital-wide infection control plan.
