Description:
The breast clinical nurse specialist role encompasses the whole of the patient’s journey for those who have or fear they may have breast cancer, providing specialist nursing advice and support for the patient and their families or carers. Providing support to ensure they have all the relevant information to enable them to make an informed choice about their treatment. To work closely with the ward staff and the wider multidisciplinary team members.
To work as part of the multidisciplinary team within the breast care service.
To support the development of nurse led services.
Work alongside members of the MDT to facilitate the breast cancer pathway and support delivery of a reduction in the number of patients waiting over 62 days for cancer diagnosis or treatment.
As a key worker to the patient you will act as their advocate in co-ordinating their care and treatment plan.
Provide an interface between patients/carers and the multi-disciplinary team, developing nursing practices and identifying opportunities to improve quality of patient care/experience.
Co-ordinate and run nurse-led access clinics and prioritise all referrals from MDT. Leading in the assessment, planning, implementation and evaluation of care.
To identify and refer breast patients for psychological/physio/dietetic support as required.
Exercise leadership and inspire colleagues to improve standards and quality to develop professional practice.
To identify strategies to help inform and educate the patient and family members and support them in decision making.
To participate in the development of the individual care pathway for the patient with established breast disease.
Undertake and facilitate nursing research and audit reporting findings to the multi professional team.
