Description:
This position provides direct therapeutic and indirect consultation Social Work services to the In-Patient Departments. The successful candidate will be responsible for the assessment, care plan development, implementation and evaluation of social work treatment plans for patients. The successful candidate will be a member of the multidisciplinary professional team and will perform all other relevant duties assigned to ensure the efficient operation of the units. The successful candidate’s home site will be the St. Catharines Site but may be required to respond to work requests at other sites.
QUALIFICATIONS:
o Bachelor’s degree in Social Work
o Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
o Two years in the past five years of working experience in an inpatient or outpatient medical / physical rehabilitation setting
o Must possess therapeutic skills to conduct psycho-social interventions, crisis intervention and brief therapy interventions
o Knowledge of and experience in case management
o Skilled in counseling and educating individuals, families and groups regarding coping strategies and cognitive behavioural change theory related to lifestyle changes.
o Knowledge of psychosocial and physiological impact of chronic illnesses and conditions, heart and stroke disease an asset
o Understanding and knowledge of available community resources
o Knowledge of all relevant legislation (i.e. Health Care Consent Act, Substitute Decision Act, Mental Health Act, Public Guardian and Trustee, Child Protection Act)
o Demonstrated professionalism and community relations skills while dealing with the public (patients and families), as well as various medical and non-medical professionals
o Evidence of effective customer service, organizational, interpersonal, communication, judgment, critical thinking and decision-making skills
o Ability to work effectively within a multi-disciplinary team
o Flexible, self-directed and able to work effectively with minimal supervision
o Consistent and regular attendance required
o Proficiency in both official languages (English & French) will be considered an asset
