We are seeking a highly motivated Business Support Officer to assist with the development and implementation of the Connecting Care Programme across Gwent.
The Connecting Care programme has been developed to replace the existing national system WCCIS (CareDirector) that will be decommissioned in Jan 2026.
It is estimated that this project will come to an end in March 2026, with potential for continuation past that point.
The Gwent region will be implementing a single regional replacement system that will be implemented across 5 South East Wales Local Authorities; Blaenau Gwent, Caerphilly, Newport, Monmouthshire.
Bringing all 5 LAs into the Connecting Care system will maximise and further enhance the collaborative working and information sharing across Gwent.
The post will be hosted by Torfaen County Council, but the post holder will be expected to work flexibly across the region, developing positive relationships in all five local authorities, with the Local Health Board and with other regional partners/stakeholders.
For an informal discussion regarding this vacancy please contact: Matt Lewis: mattlewis@srswales.com or Jason O’Brien: jason.o’brien@torfaen.gov.uk
