For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. The Clinics provide psychotherapeutic and psychopharmacological therapy, as well as individual, group, family therapy, crisis intervention, medication management, case management and collateral sessions. Our staff members follow an approach to health care that emphasizes wellness, recovery, trauma-informed care, and physical-behavioral health integration.

QUALIFICATIONS

  • NYS Licensed Professional Nurse
  • 1 years of experience working with the behavioral health Population
  • Bilingual preferred (Chinese, Korean, Russian, Spanish) but not required
  • Excellent time management, organizational and computer skills.
  • Ability to appropriately evaluate and intervene in crisis situations
  • Excellent oral and written communication skills required.
  • Ability to work flexible hours (evenings) as assigned.

Responsibilities:

  • Perform health monitoring services and assessments
  • Responsible for tracking information/ deliverables for the Cabrini grant funds.
  • Lead workshops for clients on issues related to health and wellness.
  • Facilitate coordinated care for targeted critical/high need cases.
  • Completes documentation, both paper and electronic in a timely manner.
  • Provide in-service training and be available to consult with staff.
  • Conduct and prepare assessments, case logs, documents case notes, care plans for clients.
  • Dispense and administer medications to clients (ex. Suboxone, Tylenol, etc.)
  • Monitor clients who may be experiencing precipitated withdrawal symptoms during a Suboxone induction
  • Conduct Urine Toxicolog
