Responsibilities:
ROLES & RESPONSIBILITIES:
The Nurse in the Clinical Services and Support department plays a critical role in improving patients’ lives by providing clinical and educational services. The nurse provides support to patients using the SmartMat to reduce their risk of developing diabetic foot ulcers. In a blended call center, using a combination of clinical knowledge and evidence-based practices, critical thinking, and behavior change techniques, the nurse supports the patient in their journey to optimal health.
PRIMARY RESPONSIBILITIES INCLUDE:
- Deploys a patient-centered approach that provides a superior patient experience.
- Provides training and education to the patient regarding use of the SmartMat, as well as ongoing support and coaching regarding their health.
- Engages patients to actively participate in the Podimetrics program.
- Ensure patients stay connected with their providers to improve clinical outcomes
- Conducts inbound and outbound telephonic clinical assessments to evaluate for concerning findings that require intervention by a healthcare provider, identify continuing educational or coaching needs, and to reenforce foot care best practices
- As a result of applying best practice the nurse drives outcomes for patients to self-manage conditions, be on their own feet, and lead fulfilling lives.
- Provides feedback and voice-of-the-patient experience to inform program, process, and technology improvements.
