Description:
Role and responsibilities Service Administration Oversee daily administration of the cancer care service, including running weekly EMIS searches for newly diagnosed cancer patients across all practices, contacting patients to arrange appointments, and ensuring appropriate use of appointment slots. Maintain data accuracy and presentation for submission to the PCN team along with other administrative duties. DES Compliance Collaborate with GP Practices to fulfil all requirements of the Network DES Contract related to early cancer diagnosis, including: Reviewing Cancer Referral Practices Work with partners to improve cancer referral practices and support early diagnosis. Develop a clear set of actions and milestones to improve referral practices for colorectal, lung, and one additional cancer type determined locally.
Regularly audit referrals for patients diagnosed with cancer using NCDA templates and the OHID Fingertips data tool to inform improvement actions. Review local referral practices against NICE Guideline NG12 to ensure compliance. Improving Screening Uptake Implement strategies to enhance screening uptake, particularly for breast, bowel, and cervical cancers. Analyse data from OHID Fingertips to identify variances in screening uptake and take corrective actions.
Audit non-responders to cancer screening programs to identify barriers and work with relevant partners to address these issues. Promote the Every Contact Counts approach to check screening history at every patient appointment. Utilising Direct Access Tests Promote use of the direct access tests where appropriate, ensuring adherence to NHS England guidelines. Pathway Facilitation Serve as a key liaison between general practice, acute providers, and practice clinical leads to streamline pathways for newly diagnosed cancer patients and those with existing diagnoses requiring support.
Patient Support Act as a link between patients and general practice, facilitating supportive discussions and working collaboratively with patients and GP practices to define individualised care plans. Holistic Needs Assessments Conduct holistic needs assessments (HNAs) to evaluate patients’ emotional, physical, practical, spiritual, and social needs, enabling timely interventions. Caseload Management: Manage a caseload effectively, responding to client referrals promptly as per the defined objectives of the PCN. GSF Meetings Organise and participate in Golden Standard Framework (GSF) meetings, documenting discussions and providing written summaries, including meeting objectives, actions, and outcomes to the PCN manager and practices.
Data Accuracy Ensure all patient information on EPaCCS is accurate across all practices and communicate updates effectively. Cancer Action Plan Lead the creation, update, and review of the PCN Cancer Action Plan, collaborating with practices, the PCN Manager, and the Clinical Director on objectives and achievements. Ensure alignment with the Cancer Alliance’s priorities and initiatives. Community Resource Access: Facilitate patient access to local services, including physical activity, dietary support, emotional care, and information resources, to enhance treatment and recovery outcomes.
Patient Empowerment Empower individuals to understand and manage their treatment options and lifestyle changes for improved health and well-being. Safeguarding Procedures Identify and raise patient-related concerns following safeguarding protocols. Clinical Data Input Directly input clinical data into the EMIS system for HNA assessments while ensuring compliance with information governance. Proactive Communication Engage in proactive communication with patients to promote health and well-being, including referrals to social prescribing and physical activity services.
Training and Development Attend mandatory training and pursue personal and professional development relevant to the role. Service Evaluation Contribute to service evaluation, data collection, and implementation of service improvements in alignment with DES metrics and targets. Collect and analyse feedback from patients and stakeholders to inform service delivery. Cancer Strategy Updates Stay informed on National/Local Cancer Strategies and ensure the application of the latest guidelines in practice.
Additional Duties: Undertake other reasonable duties as required within the roles general scope. This role is essential for providing high-quality care coordination services that enhance the overall patient experience within the healthcare system
