Description:

We are recruiting a Cancer Clinical Care Coordinator to work within the Nurse Specialist team who shares our vision to be trusted to provide consistently outstanding care and exemplary service to our patients.
As part of a 2-year Pilot, the Macmillan Cancer Care Coordinator (MCCC) will work alongside the Clinical Nurse Specialists in Skin and Lung cancers to ensuring a safe and efficient service for patients. You will contribute towards the provision of providing a high standard of evidence-based care, supporting the management and supervision of patients throughout their treatment pathway.

Provide a seamless service to patients with proximal supervision from the tumour specific Clinical Nurse Specialist (CNS).
Coordinate care by providing a single point of access, including rapid re-entry into the system for those patients identified as having urgent or specialist needs.
Communicate effectively within the multidisciplinary team and other departments to ensure that patient care is well planned and coordinated.
Have contact face to face and telephone communications with patients, carers and health care professionals.
Contributes to the improvement of services
It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies to be determined within the clinical team with the focus on patient needs.

In addition, you will receive a High Cost Area Supplement of 15% of your basic salary, subject to a minimum payment of £4,314 and a maximum payment of £5,436 per annum pro rata.
