Description:

We are looking to appoint a highly motivated and enthusiastic cardiac rehabilitation exercise specialist with excellent communication skills to join our experienced, friendly and dynamic multidisciplinary cardiac rehabilitation team.
The role will involve working in the community alongside cardiac rehabilitation specialist nurses, other MDT members to assist in assessing, educating and delivering physical activity sessions to patients with cardiovascular disease.
You will support the cardiac rehabilitation specialist nurses in providing and supervising a comprehensive cardiac rehabilitation programme from referral to discharge, adapted to individual clinical need and patient preference, for all cardio-vascular patients meeting the referral inclusion criteria.
It is essential that you hold a full UK driving licence and preferably have access to suitable vehicle for business purposes.

Conduct screening and a comprehensive physical activity assessments including baseline assessment of fitness/functional capacity and risk stratification for all patients recruited to the programme in line with BACPR standards.
Lead and co-ordinate the supervised Phase 3 exercise sessions.
Facilitate behaviour change for all components of the cardiac rehabilitation programme using evidence-based health behaviour change theory to increase the probability of successful long-term behaviour change.
Provide physical activity behaviour change support for patients (and significant others), including goal-setting and pacing skills to support long term self- management.

The post holder will work with the Community Cardiac Team, and specifically with the Cardiac Rehabilitation Nurses and existing Exercise Professional to continue shaping the future delivery of cardiac rehabilitation care across Greenwich and Bexley.
Key Task and Responsibilities
To have comprehensive knowledge of the British Association of Cardiac Rehabilitation (BACPR) guidelines for best practice and ensure their implementation in the community rehabilitation service.
To improve patient and carer confidence to participate in decisions about their care and manage their condition effectively to prevent the development of their cardiac condition (also known as ‘patient activation’).
Conduct screening and a comprehensive physical activity assessments including baseline assessment of fitness/functional capacity and risk stratification for all patients recruited to the programme in line with BACPR standards.
Lead and co-ordinate the supervised Phase 3 exercise sessions.

Excellent IT skills, with ability to produce reports, and interpret data, for example Key Performance Indicators as it relates to the service. Ability to be mobile, flexible to do home visits as appropriate (in line with the Trust’s lone workers policy) and attend meetings at local level in a timely manner, ensuring best practice is learned and introduced into the local service for service improvement purposes.
Provide an environment that encourages client centred involvement where clients are facilitated to ask for help, advice and education.
Ability to prioritise and evaluate own workload
Liaise closely with our local providers to provide exit route opportunities and encourage long term sustainability of an active lifestyle.
Knowledge and understanding of the NHS Long-Term plan, NICE guidance and AACVPR standards
