Description:

We are currently seeking a highly organised and proactive individual to join our team as a Cardiology Tracker. The primary purpose of this role is to monitor, reporting, and track cardiology patients and liaise with members of the multidisciplinary team.

As a Tracker, you will work closely with both clinical and administrative staff, as well as other health and social care providers, to manage patients journey. Your main duties include monitoring, reporting, and tracking patients to ensure timely and efficient processing of their journeys. You will resolve pathway delays promptly and escalate issues as needed.
Additionally, you will support the MDT Coordination Team by providing administrative assistance to the cardiology teams, ensuring all necessary paperwork and clinical information is available. You may also attend MDT meetings to record attendance and outcomes. Ensuring the efficient transfer of patients and information between trusts is critical.
You will be involved in whiteboard meetings, working with discharge planning and transfer of care teams and manging the patient discharge pathway. You will work closely with the senior site team and capacity team as well as teams from other hospitals regarding the safe and timely transfer of patient care.
