JOB SUMMARY:

Provide befriending and outreach services to the elderly at a designated service boundary. Work closely with the Centre Supervisor in the befriending care plan implementation and monitoring of clients’ condition.

JOB DESCRIPTION

• Conduct community outreach and relevant need assessments to the elderly at a designated service boundary.
• Conduct care befriending services to frail and vulnerable elderly at their homes.
• Provide referral to help services for the elderly in needs.
• Recruit volunteer befrienders to assist with the outreach and befriending services.
• Train volunteer befrienders to provide monitoring and appropriate referral to support services.
• Perform administrative and reporting duties such as visitation notes, records and other necessary data and statistics required by the authorities.
• Perform other duties as assigned by the reporting officer.

JOB REQUIREMENTS

• Minimum Diploma Holder.
• 1 to 2 years of relevant experience in elderly befriending service.
