Description:
Work with administrative and clinical staff to help improve services to our patients and help to implement improvements. Manage the cervical smear invites and cervical smear system. Develop and implement interventions to decrease barriers to accessing primary care services for people facing health inequalities Liaise with GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN. This patient population includes but is not limited to persons who are unable to leave their homes or require significant assistance to leave the house due to illness, frailty, surgery, mental ill health or nearing end of life.
Managing any necessary meetings to support care planning, identify patients for discussion, organising the meeting and circulating required information beforehand as necessary Ensure that meeting actions are recorded, disseminated and followed up in a timely way; so relevant practitioners are aware of meeting decisions and actions / outcomes, and chase for action resolution and update Take minutes/notes of meetings if required during MDT meetings Undertake regular reviews of the personalised care and support plans developed with patients Work in line with national best practice when developing personalised care and support plans Work with patients over the phone, in person in the practice or for those who are housebound - carry out home visits as necessary. Participate in Phlebotomy (Adults only) Participate in Adult immunisations Seasonal Influenza, Pneumococcal, B12, Covid-19, Shingles Check patient height and weight measurements Check patient blood pressure and pulse checks and escalate any concerns to clinical staff Provide yearly health reviews for patients with Learning Disabilities Support delivery of these care plans by co-coordinating input from a range of different professionals and services, and helping patients and their carers/family to navigate across health and social care services Work as part of a team, coordinating care between GPs, practice nurses, clinical pharmacists, social prescribing link workers and health coaches Please see job descritpion for full details
