REQUIREMENTS:

  • Minimum of 5 GCSEs
  • Excellent time management, interpersonal and organisational skills
  • Knowledge of EMIS
  • Minimum of 2 years administration experience in any setting
  • Experience working in primary care or within a PCN

  • Digital Transformation (Change Management and embedding digital technologies and pathways across our services);
  • Population Health improvement services, e.g., Vaccination uptake, LTC reviews, HIV screening etc.;
  • Community Dermatology Service, led by our amazing GPwER;
  • Long Covid Service - we are a local pioneer in developing this service in the community for our local populations.

    Location: Marvels Lane Clinic, London, SE12 9PN.
    Hours: 37.5 per week, Monday - Friday, 10:30am - 6:30pm.

Responsibilities:

  • Providing coordination and navigation for people and their carers across health and care services, helping to ensure patients receive a joined-up service and the most appropriate support.
  • Facilitating and ensuring the effective delivery of patient-centred, personalised health and social care plans for patients, monitoring progress and reporting outcomes, contributing to patient reviews and care planning within appropriate time frames.
  • Work closely with all relevant care agencies (primary care, secondary care, community services, Mental Health, Social Services, Ambulance Service, Voluntary Services, and other relevant service providers) to ensure a coordinated patient care plan, without requiring a further referral from the GP.
  • Acting as the single point of contact for the Practice and establishing systems and processes which will ensure a timely and appropriate response to queries from clinicians and other stakeholders.
  • Scheduling appointments, managing patient booking systems, and ensuring an effective flow of patients.
  • Handling administrative tasks, such as updating patient records, verifying personal details, and processing paperwork.
