Description:

Procare Healthcare is recruiting for a Care Coordinator to join our growing team. As part of the health care team you will ensure that clients receive the highest standards of care and support through effective allocation of staff. You will direct, mentor and supervise the team to ensure quality standards are maintained and that care is delivered in a timely way in line with client’s individual needs and wishes.

Key Responsibilities:

  • Conduct a comprehensive assessments of clients’ care needs and create individualized care plans in consultation with clients and their families.
  • Collaborate with clients, their families, and health professionals to develop and update care plans as needed.
  • To be accountable to the Registered Manager, ensuring that they are kept informed of any concerns or changes to the health and wellbeing of clients.
  • Responsible for resourcing for new packages and building on existing packages.
  • Promoting dignity, respect, and choice of clients, ensuring that the care teamwork within this ethos at all times.
  • To always ensure the emotional and wellbeing of clients.
  • Supervise and support a team of care givers, providing guidance, training, and direction, spot check carers ensuring the highest standards of care are maintained.
  • Oversee the care team ensuring staff development through supervisions, appraisals, spot checking, identifying good practice and reporting concerns.
  • Support and mentor new members.
  • Act as an advocate for clients, ensuring their voices and preferences are heard and respected in all aspects of care planning and delivery.
  • Monitor and evaluate the quality of care provided, making necessary adjustment to care plans and services as needed to maintain the highest level of client satisfaction.
  • Maintain open and transparent communication with clients, their families and healthcare professionals to ensure the best care outcomes.
  • Maintain accurate and up to date client records, including care plans, assessments, progress notes and any relevant documentation required for compliance.
  • Be available to provide care services to clients in an emergency, arrange emergency cover when required, and cover all/ any calls reasonably
  • Be available for on call support outside of regular business hours.
  • To act as a role model and be exemplar to the care team at all times.
  • Ensure all operations are Company and CQC compliant.

Requirements:

  • Ideally NVQ/QCF level 3 in care.
  • At least 1 year experience working in home care as a coordinator or senior care assistant.
  • Knowledge of CQC Compliance is essential.
  • Committed to helping improve the quality of life of vulnerable people.
  • Being flexible around working schedules, roles and responsibility.
  • Full Driving Licence and access to a vehicle is essential.

Job Type: Permanent, Full time.
Salary : £24,000.00 – £26,000.00 per annum.
