ABOUT THE CANDIDATE

If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have the following skills, experience and qualifications:

  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
  • Ideally you will have a Diploma Level 2 in Health and Social Care, or an equivalent qualification.
  • Strong listening and communication skills.
  • Familiar with local resources and services, including how to access them.
  • Motivated by helping people, with care and empathy.
  • Able to deliver person-centred support in a non-judgmental way.
  • Experience of completing holistic person-centred care planning assessments.
  • Strong IT Skills.
  • Cross site working.

Located in Shoeburyness the surgery provides a range of NHS primary care services and we aim to provide an effective, efficient and fair health care service to all our patients. Our professional team work extremely well together, we have monthly clinical training sessions that are held here on site, these are open to all levels of the clinical team.
In addition to ongoing training, we provide the NHS pension scheme which is a defined benefits scheme - 20% employer contribution.

Responsibilities:

North Shoebury Surgery has an fantastic opportunity for a care co-ordinator to join our team, on a full time basis for 37.5 hours per week.
This role involves working very closely with the practice clinical team within the partnership and is pivotal in ensuring that all patients receive the best possible care and service.

The main duties of the role include:

  • supporting the clinical directors in coordinating all key activity, including access to services, advice, and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
  • supporting digital initiatives.
  • responsibility for the co-ordination of the patients’ journeys through primary care.
