If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.
The ideal applicant will have the following skills, experience and qualifications:
- Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
- Diploma Level 2 in Health and Social Care, or an equivalent qualification (desirable not essential).
- Familiar with local resources and services, including how to access them.
- Motivated by helping people, with care and empathy.
- Able to deliver person-centred support in a non-judgmental way.
- Experience of completing holistic person-centred care planning assessments.
- Strong listening and communication skills.
ABOUT US
- We are a friendly, supportive, democratic, well organised and enthusiastic primary care network, dedicated to providing traditional family orientated general practice based in a new purpose built single site health centre in Harlow for the PCN.
- The PCN has over 40,000, patients from three practices. Lister Medical Centre based at Lister House, Staple Tye and The Ross Practice & The Hamilton Practice based at Keats House, Bush Fair.
- Innovative team structure, including social prescribers, care coordinators, clinical pharmacists, ECPs, advanced practitioners, general practice assistant and an adult mental health worker.

Responsibilities:
Harlow South Primary Care Network has an opportunity for a care co-ordinator to join our team on a full-time basis 37.5 hours per week over 5 days.
The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients. We look forward to the addition of a care coordinator to the PCN and would be able to support someone new to the role providing support and training as needed.
The main duties of the role include:
- Working with GPs and other professionals within the practice to identify and manage a caseload of patients. Particularly working with vulnerable patient groups with long-term conditions that may require this extra support. This may include working with individuals, their carers, families and healthcare teams.
- Using in-depth knowledge of locally available support and services to develop care plans with patient, focusing on prevention.
- Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
- Helping patients prepare for, or follow-up after, clinical conversations.
- Supporting digital initiatives.
- Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date.
