Responsibilities:

As part of our planned service development across Hounslow, Ealing, and Hammersmith & Fulham, we’re looking for a dedicated, creative, and hardworking Band 6 Care Coordinator for a 12-month fixed-term contract. You will play a pivotal role in delivering our community rehabilitation pathway, guiding service users to achieve their recovery goals.
Work closely with service users with complex psychosis, helping them maximise their independence in the least restrictive setting possible.
Supportive Planning: Develop detailed Care Programme Approach (CPA) plans, risk assessments, and health and wellbeing plans.
Evidence-Based Interventions: Provide tailored interventions in both individual and group settings.
Quality Improvement: Take part in, and potentially lead, quality improvement initiatives and research.
