Description:
Support the Clinical Directors and team to deliver the Directed Enhanced Services specifications for individual services. Co-ordinate the work of healthcare professionals and non-clinical staff including volunteers, keeping an up-to-date database of contacts. Provide access for patients, often being the first point of contact for families, carers, and partner organisations to answer any queries. Provide advice and information ensuring health and care planning is timely, efficient and patient-centred.
Help create single personalised care and support plans. Ensure all relevant, useful and up-to-date information is available to patients/clinicians. Support wider team with Quality Improvement projects and achieving KPIs (QOF and IIF). Monitor and cleanse existing data.
Ensure data is recorded and coded accurately and aligned within the PCN. Understand and provide support for digital initiatives. Develop relationships with external 3rdparties that help support care in patients homes and their community. To co-ordinate Multi-disciplinary Team meetings, taking notes and record actions.
Reviewing the patients needs and help them to access the services and the support, including onward referral to local services. To build and develop relationships with partner organisations, such as community services, volunteer groups, community learning disability teams, care homes and keep a comprehensive and up to date database of contacts. To support and develop a formalised process of delivering healthcare reviews to patients. Attend monthly meetings to discuss good practice and share information.
Maintain patient appointment lists and monitor attendance. Use practice level reports to identify suitable cohorts of patients to deliver personalised care
