JOB TITLE: Care Coordinator
LOCATION: Onsite, CUPS - 1001 10 Ave SW, Calgary,
REPORTS TO: Team Lead, Care Coordination and Intake
SCHEDULE: Full-time, Mon to Fri, 8:00 am-4:00pm
CONTRACT TYPE: Term Role to December 2026
