JOB DESCRIPTION:

As a Care Coordinator, you will be responsible for overseeing and coordinating the care provided to our clients. Your main duties will include but not limited to :

  • Managing a team of care staff and ensuring high-quality care delivery.
  • Conducting assessments and creating care plans tailored to individual client needs.
  • Liaising with clients, families, and healthcare professionals to ensure smooth service delivery.
  • Monitoring and maintaining records of care schedules and client progress.
  • Providing support and guidance to care staff, ensuring compliance with policies and procedures.
