JOB DESCRIPTION:
As a Care Coordinator, you will be responsible for overseeing and coordinating the care provided to our clients. Your main duties will include but not limited to :
- Managing a team of care staff and ensuring high-quality care delivery.
- Conducting assessments and creating care plans tailored to individual client needs.
- Liaising with clients, families, and healthcare professionals to ensure smooth service delivery.
- Monitoring and maintaining records of care schedules and client progress.
- Providing support and guidance to care staff, ensuring compliance with policies and procedures.

- Previous experience in a care coordination or management role.
