Description:
This is an exciting opportunity for an experienced mental health worker to join Trafford early intervention service.
This is an exciting opportunity for an experienced Mental Health Nurse to join our specialist service for people aged 14-35 experiencing a first episode of psychosis.
This post is part time. 30 hours per week.
Based on The Department of health implementation guidelines Early Intervention Services:
Reduce the stigma associated with psychosis.
Improve professional and lay awareness of the symptoms of psychosis and the need for early assessment.
Reduce the length of time young people remain undiagnosed and untreated.
Develop meaningful engagement, provide evidence based interventions and promote recovery from the outset.
Increase stability in the lives of service users, facilitate development and personal fulfilment.
Provide a user led service, i.e. a seamless service available for those from age 14 to 35 years (on acceptance) that effectively integrates child, adolescent and adult mental health services and works in partnership with primary care, education, social services, youth and other services.

The role of the care coordinator is to coordinate an evidence based care plan to meet the needs of the service user and their loved ones.

v To manage a caseload of people with complex mental health and social care needs, using evidence-based practice to assess, plan, implement and evaluate interventions and document these in accordance with local and national standards and guidance.
v To have care-coordination responsibilities for an agreed number of clients on the first episode and extended assessment pathway within the team.
v To participate in the assessment of people referred to the team with possible first episode psychosis.
v To participate in the operational systems and the development of the Early Intervention team.
v To provide leadership for junior staff, multi-disciplinary support staff and students through formal supervision, education and appraisal
