Job Description
In this role, you will perform a variety of duties in the coordination of scheduling services whilst providing the highest quality of services to clients. Responsible for managing the needs of clients and working alongside the Deputy Care Managers and supporting the Registered Care Manager.

Overview:

  • Create and manage schedules tailored to clients’ needs and Care Professional availability.
  • Respond promptly to schedule changes, ensuring smooth communication with team members.
  • Foster strong relationships with clients, families and care professionals to enhance experiences.
  • Setting up of care packages from assessment stage to onboarding.
  • Collaborate with the Registered Care Manager & Recruitment Manager to maintain staffing levels and meet care demands.
  • Utilise electronic scheduling systems for accurate record-keeping.
  • Perform additional duties as necessary for business success.
  • Participate in the out-of-hours on-call rota.
  • Provide contingency care cover as needed.

Coordinate and monitor client/care professional activities:

  • Respond to changes in clients needs.
  • Update records on relevant operational systems.
  • Manage client onboarding / terminations and related tasks including setting up and organizing the relevant client files.
  • Establish recurring care schedules.
  • Assisting the Deputy Care Managers to create weekly plans to manage the needs of the business.
  • Review and update Client events.
  • Update relevant systems with client tasks/medications as required.
  • Maintain accurate client records.
  • Review and update care plans as needed, working together with the Deputy Managers.
  • Liaise and collaborate with healthcare professionals such as GP’s, District Nurses, Occupational therapists etc.
  • Communicate with the Care Professionals with any client updates etc as needed via the correct communication channels.

Compliance and Documentation:

  • Ensure compliance with all relevant regulations, policies, and procedures and Franchise Standards
  • Maintain accurate and up-to-date documentation, including client records, care plans, and incident reports etc.
  • Review/ audit the live client visit notes on relevant systems.
  • Update relevant systems with client tasks/medications etc as required.

QUALIFICATIONS

  • Good working knowledge of IT systems with experience in Microsoft Office or Google Suite and virtual communication platforms is essential. Must have the aptitude to learn and adopt new technologies and software where appropriate.
  • Prior scheduling experience in home care or similar environments.
  • Level 2/3 NVQ in Health and Social care or equivalent desired but not essential.
