Right at Home, Adelaide Region are seeking qualified, reliable, and passionate Care Coordinator ( Prefer Registered or Enrolled Nurse) . The Ideal Candidate: The ideal candidate for our team will have at least 2 years experience with managing client enquiries, client consultations, client onboarding, and a client caseload for Home Care Package, NDIS, and private pay clients. The care coordinator will have experience managing the complex scheduling requirements for community care workers.
Purpose of Position: This position places a strong emphasis on prioritizing customer satisfaction and involves engaging with families, conducting in-home care assessments, and formulating customized care plans tailored to meet the specific needs of our clients. The role also encompasses supporting administrative personnel in various aspects such as recruitment, training, and overseeing the performance of our caregivers and nurses.

QUALIFICATIONS:

  • Possession of either current AHPRA registration (RN/EN) or a Certificate III/IV in Individual Support (specialized in Ageing or Disability ( community/ Home care Package) & Preference will be given to those with RN/EN certification).
  • Current Australian (Satisfactory) National Police check, Working with Children Check,
  • Current First Aid & CPR certificate.
  • Driver’s license and your own reliable registered vehicle

  • Excellent verbal and written communication skills
  • Professional Indemnity Insurance (RN/EN)
  • A smart mobile phone (iPhone / android)

PREFERRED SKILLS :

  • At least 2 year experience as a scheduler, case manager, care coordinator ( Home Care Package ), hospital discharge planner, or NDIS support coordinator
  • Solid knowledge in the Disability and/or Aged Care sector(s)
  • Must be Registered Nurse / Enrolled Nurse.
  • The ability to operate efficiently and deliver excellent service when under pressure.
  • Clinical skill in wound care
  • Strong time management and organizational skills to manage competing priorities effectively.
  • Relationship development skills to support diverse clients to achieve their goals and objectives.
  • Familiar with Visual Care
  • Intermediate to Advanced Microsoft Office experience (Excel, Word, Outlook, and CMS)

Responsibilities:

  • Assisting with handling high volume of incoming telephone and internet enquiries.
  • Collaborate with the scheduler to efficiently schedule services for our clients, taking into account the qualifications and availability of care workers. Assist in the creation of master schedules and the management of daily updates and adjustments to existing schedules.
  • Meeting with clients and assessing their needs. This role will handle anything from information requests, feedback, general enquiries to complaints.
  • Preparing in-home care assessments and drafting care plans to best meet client requirements.
  • Training, and managing the performance of the care team.
  • Maintaining client and care worker compliance and documentation according to government standards.
  • Ensure all documentation is accurately and comprehensively completed in Right at Home databases and software programs in accordance with organisation policies and procedures and legislative frameworks.
  • Providing “on-call” services on rotation after hours, on the weekends, and on public holidays.
