To succeed in this role, you will have:
- A current registration as a Registered Nurse with AHPRA
- Exceptional understanding of the ABC’s in relation to the Aged Care Quality Standards
- Knowledge of home care packages
- A Comprehensive knowledge of clinical issues of ageing, including dementia related disorders and management of challenging behaviours
- Working knowledge of AN-ACC
- Strong interpersonal, communication and customer service skills

Based at our beautiful Nellie Melba Retirement Village, as a Care Coordinator you’ll provide tailored care solutions that empower residents to thrive independently, enhancing their overall quality of life. Your focus will encompass managing the serviced apartments and a number of independent living residents who benefit from our care packages and be required to lead the clinical delivery of services.
As the primary liaison, you’ll cultivate robust relationships with residents and families, fostering a nurturing environment.
Your responsibilities will include:
- Delivering compassionate, high-quality care across our village.
- Coordinating a spectrum of care services, including allied health, housekeeping, and companion visits.
- Complete thorough and timely documentation to ensure seamless care delivery.
- Providing invaluable assistance to your Clinical Manager and Case Managers, ensuring operational excellence and continuous improvement.
We have a 12 month Maximum Term role due to internal movement, working Tuesday to Saturday 0800 - 1600. The successful candidate will be agile, positive and have strong administration skills and ideally available to start late September to receive a comprehensive handover.
