Care Coordinator - £28-35K
An opportunity to join a market leading company and provider of care services to the community in a newly created permanent role.
A Durham based business they provide supported living, mental health, personal and respite care as well as assisting those with learning disabilities.
The successful candidate must have their own transport and be prepared to travel across the County Durham area. You will have a strong understanding of care planning and scheduling, excellent organisational and multi tasking skills, be proficient in the use of care management systems as well as MS Office and knowledge of CQC standards.
Key Responsibilities:
Client Care Management
Staff Co-ordination and Support
Compliance and Administration
Communication and Team work
NVQ in Health and Social Care (or equivalent) is highly advantageous
Job Types: Full-time, Permanent
Pay: £28,000.00-£35,000.00 per year
