Description:
Care Coordinator
We are a compassionate and dedicated team based in Waterlooville. Our mission is to provide exceptional care and support to individuals living in the community. We are currently looking for a caring and organised Care Coordinator to join our team. The Care Coordinator will play a vital role in coordinating care services for our clients, ensuring they receive the support they need to thrive independently.
- Office hours: Mon-Fri 9am-4pm
- Cover Out of hours on call: on a rota basis
Responsibilities:
- Conduct client assessments & risk analysis to determine clients’ needs and develop personalised care plans - using templates
- Monitor clients’ progress and update care plans as needed - maintain accurate records
- Collaborate with caregivers, healthcare professionals, and family members to ensure comprehensive on going care for clients
- Deal with daily concerns / queries from carers & clients
- Report safeguarding issues
- Carry out back to work meetings with staff following sickness
- Manage disciplinaries
- Carry out mini inductions for new staff
- Reconciliation of call visits
- Cover care visits during office hours at short notice due to sickness
- Support scheduler with last minute changes
- Support senior staff
- Cover Out of hours on call: on a rota basis
Qualifications:
- Experience of working in the Health & Social Care Industry
- NVQ Level 3 in Health & Social Care
- Proficiency in IT systems
Skills:
- Excellent communication skills, both verbal and written
- Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines
- Demonstrate flexibility for the needs of the business
- Confident & professional manner
- Compassionate attitude and genuine desire to help others, building strong relationships
