Description:
We are seeking an experienced Care Coordinator to help continue to develop a bespoke Community Care/ Support Service across the Buckinghamshire area. The Care Coordinator provides and coordinates individualized care and resources to clients so that they may continue to receive care within their own homes and within the community. The role is extremely fast-paced and therefore requires someone who is able to remain calm and positive whilst working under pressure and to deadlines.
Based in our office in Amersham, working Monday to Friday core hours of 9am-5pm, with weekend availability needed for on call cover, the successful candidate would also have a good knowledge of the Bucks area.

Responsibilities:
- Ensure care is provided to our clients according to all relevant policies, procedures and regulations
- Be accountable for the allocations of carers rosters to ensure all our clients’ needs are met
- Provide assessments to potential clients to establish their individual care needs
- Reassessments of existing clients and review current care plans
- Liaise with all family, medical, professional and other resources as required
- Provide excellent customer service levels to all internal and external users of the business
- Carry out timely spot checks, supervisions and appraisals of care staff
- Provide on call cover out of hours service cover, working as part of a rotated team
- Maintain excellent and accurate communication between the workforce and management
- Punctuality of the service
- Continuity of care staff team
- Identify and provide solution focused feedback on reporting mechanisms
- Effective use of procedures and systems
- To proactively promote equality, diversity and inclusion
- Ensure that you manage your staff hours and rotas effectively, including ensuring that staff have appropriate breaks and rest periods, have allocated travel time, are working with minimum / maximum weekly hours and electronic timesheets are correct
- Staff recruitment files are up to date post interview
- Work flexibly to meet the operational needs of the Homecare service and deliver care in the community if required
- Pick and drop staff to one service user to another if needed.

Pay: £23,500.00-£24,000.00 per year
