Horninglow Bungalows is a supported living complex for up to 16 tenants. The service has a strong ethos of maximising independance and supporting the tenants in their own homes. The team support people to live an ordinary life which includes accessing their community, maintaining connections with family and friends and supporting people with their health and wellbeing.

Main Responsibilities
As a Care Coordinator, you will manage the team and undertake support duties to deliver the day-to-day operations of the service, ensuring an excellent level of care to enable the people we support to have a comfortable and dignified life, where they are empowered to achieve their ambitions. As the most senior person on duty, you will be responsible for resolving problems and ensuring the smooth running of the service. You will manage a team comprising of Senior Support Facilitators, Support Facilitators.
The role includes acting as a Key Worker for the people we support, creating and reviewing care plans and risk assessments, overseeing medication administration, managing staff, and quality assurance and compliance.
You will be expected to work flexibly across the week according to business need, undertaking a range of shifts that may include weekend, and bank holiday working, early morning, evenings, split shifts, and sleep-in duties. Rotas are subject to change depending on business need.
Although this role is based primarily at Horninglow Bungalows in Burton on Trent, there may be times when you are expected to work flexibly across Provider Services, including different care settings, delivery approaches, and locations, as required.
The Ideal Candidate
What you will bring
You are enthusiastic about working with people with learning disabilities and autism and are able to be caring, sensitive and patient while supporting people to be as independent as possible. You have good communication and interpersonal skills, including the ability to communicate effectively with staff at all levels, build good relationships with relatives and other professionals and the ability to relate positively to the people we support.
You are able to manage, motivate, coach and support staff and nurture a team to reach their potential.
You have sound judgement and the ability to resolve day-to-day problems, emergencies, and issues. You have conflict resolution and negotiation skills and can remain calm in challenging situations. You are able to adapt and respond positively to change and work effectively under pressure and prioritise workloads.

What you will have

  • NVQ level 2 qualification in a relevant discipline
  • Experience with administering medication
  • Knowledge and understanding of the needs of people with learning disabilities
  • Knowledge of health and safety requirements in social care settings, including safe staffing ratios
  • Knowledge of the legislative and regulatory frameworks affecting vulnerable adults
  • Knowledge of care planning and reviews and evidence of a commitment to achieving positive, personalised outcomes for individuals
  • Knowledge of the principles of person-centred support, active support model and strengths-based approaches
  • Knowledge of risk management and positive risk taking and experience assessing, managing, and mitigating risks
  • Experience in the management of staff, including supervision and performance management
  • Experience of promoting genuine community participation and inclusion for people with care and support needs
