Care Coordinator

a)To be the point of contact for service users and their families/carers where information on their short-term care support is required, where they wish to make enquiries about; anticipated visits and or actions of team members or to raise concerns about their short-term care support provided. b) To sign-post referrals to team members and other services as appropriate. Monitors, tracks and reports on progress within the patient pathway. Escalate any issues or delays to relevant Team Lead for appropriate action.
c) To process and administer referrals made to short-term care according to the relevant health criteria seeking advice from relevant team members when required. d) To provide a client centred approach to ensure all clients and carers views and opinions are considered and all people who make an enquiry are given respect, dignity and understanding. e) To request completion of an initial holistic assessment by a relevant MDT team member.
