THE OPPORTUNITY:

We are seeking a Care Management Specialist to support and ensure holistic wrap around case management support within the Cornerstone Family Services team. YSM’s Cornerstone Care Management program provides restorative and preventative support to families involved with Child Welfare through an Anti-Black Racism (ABR) lens so that families become community connected, independent, stable, healthy units.
Reporting directly to the Care Management Supervisor the successful candidate will utilize YSM’s RISE Model (Respond, Invite, Support, Engage) to provide a full wrap-around system of support through partnerships to ensure that families move from crisis to stability and out of poverty:

QUALIFICATIONS:

The successful candidate will have the following skills and training:

  • MSW/BSW/SSW or equivalent certifications with minimum 2 years’ experience
  • Able to work with diverse people groups within a multi-faith, multi-cultural, anti-oppressive framework
  • Ability to make analytical decisions around case management and program delivery
  • A demonstrated passion for poverty alleviation
  • Excellent written and oral communication skills
  • Must have strong administrative and computer skills
  • Skilled at preparing, delivering and facilitating workshops
  • A police check will be required
  • First Aid & CPI training is an asset
  • Demonstrated personal commitment to YSM’s mission, principles and faith-based values

Compensation: YSM offers market competitive rates and will pay a starting salary of $52,443.28 for this role, combined with a comprehensive suite of benefits and perks.

Responsibilities:

  • To provide holistic support to families with a focus on young families (parents under 30) and those involved with Child Protective Services.
  • To manage a caseload of 20 families
  • To co-lead trauma-informed psychoeducational groups 2-3 times years
  • To periodically deliver workshops that are relevant to building capacity
  • To work collaboratively with parents and their dependents to address multiple barriers that threaten family stability in order to mitigate admission
  • To provide preventative measures to families under investigation for childcare services.
  • To ensure that all interventions incorporate: trauma-informed, Anti-Oppression and Anti-Black Racism (ABR) lens
  • To provide unbiased, objective observation and documentation for families receiving parental access visits.
  • To oversee social work student placements
  • To partner with internal and external agencies
