QUALIFICATIONS & EXPERIENCE

  • Knowledge and experience of community-based care and support services
  • Great customer service manner
  • Strong administration skills in Microsoft Office & Database experience
  • Highly efficient and organised with proven time management skills

Responsibilities:

ABOUT THE ROLE:

Home Instead Peel & Southwest is currently hiring a Care Manager located out of our Mandurah office. This role will be responsible for managing your own client base, ensuring services being delivered are those of quality. The Care Manager is also responsible for completing comprehensive care reviews to develop care plans in-line with funding allowances while developing and implanting strategies for efficiency of care.

KEY RESPONSIBILITIES

  • Visit your clients and families on a regular basis to review the services required
  • Work alongside the rostering team when allocating the best suited CAREGiver for the service, taking into consideration the client’s preferences and tasks required
  • Monitor and respond to incoming service feedback reports
  • Meet and introduce the CAREGiver to each client’s very first service
