Responsibilities:
THE ROLE:
We are looking for a Care Planner who are passionate about a person-centred approach to care, to join our wonderful facility in Yackandandah in the North East region of Victoria.
The Care Planner promotes safe, high-quality care and accurate recurrent funding for residents by ensuring care plans are accurate and up to date. The Care Planner is responsible for assessment, care planning and care evaluation in accordance with company and resident policies and works closely with the Care Manager and Care Team to achieve their objectives.
WHAT YOU WILL DO:
- Complete interim assessment and care planning on day of admissions
- Complete quality care evaluations
- Ensures care plans are developed in true partnership with residents
- Liaise with the company revenue and classification team (ACF1/AC ACC) to ensure accurate classification and optimise recurrent funding
- Provision of safe, high-quality care in accordance with the Aged Care Quality Standards
- Positive resident experience through resident-first approach
