QUALIFICATIONS:
Required:
- LCSW with at least 1 year experience in health care or LSW under the supervision of an LCSW
SCHEDULING REQUIREMENTS:

Essential Functions:
- Assesses and evaluates each caregiver’s personal history and that of the person being cared for
- Completes all system mandatory modules for employees
- Individualizes a care plan based on evaluation of all aspects of a client’s current needs for support and referral to appropriate agencies for assistance
- Promotes program services to all health care providers in Ocean County
- Properly records all elements of visit on client’s chart
- Research any new resources for caregivers in Ocean County,
- Schedules appointments at convenient times for the client, including time for phone session if unable to come to center
- Provides psychoeducation on disease process and behavioral changes
- Counseling to develop and enhance coping skills, communication techniques, and advocacy skills
- Counseling for care planning and decision-making
- Facilitates weekly Caregivers Support Group,
- Appropriately records all information on clients seen for monthly/quarterly grant reports
- Performs other duties as required by the department
