QUALIFICATIONS:

Required:

  • LCSW with at least 1 year experience in health care or LSW under the supervision of an LCSW

SCHEDULING REQUIREMENTS:

  • Per Diem Scheduling

Essential Functions:

  • Assesses and evaluates each caregiver’s personal history and that of the person being cared for
  • Completes all system mandatory modules for employees
  • Individualizes a care plan based on evaluation of all aspects of a client’s current needs for support and referral to appropriate agencies for assistance
  • Promotes program services to all health care providers in Ocean County
  • Properly records all elements of visit on client’s chart
  • Research any new resources for caregivers in Ocean County,
  • Schedules appointments at convenient times for the client, including time for phone session if unable to come to center
  • Provides psychoeducation on disease process and behavioral changes
  • Counseling to develop and enhance coping skills, communication techniques, and advocacy skills
  • Counseling for care planning and decision-making
  • Facilitates weekly Caregivers Support Group,
  • Appropriately records all information on clients seen for monthly/quarterly grant reports
  • Performs other duties as required by the department
