This job provides direct supervision to registered nurse case managers and social workers in accordance with organizational standards. Oversees the provision of services, demonstrating commitment to the agency’s mission, training new staff, ensuring team cohesion, and reviewing staff documentation.

EDUCATION
Required -Bachelor’s degree in nursing or other healthcare discipline
OR
Master’s degree in Social Work (MSW).
Preferred – Master’s degree in Nursing (MSN)
OR
Licensed Certified Social Worker (LCSW).
WORK EXPERIENCE
Required - 3 years of utilization and/or case management experience with a registered nurse license
OR
Master of Social Work (MSW).
Preferred - 3 years of supervisor or leadership experience in a hospital environment.
KNOWLEDGE SKILLS AND ABILITIES
- Leadership skills and ability to foster an environment of team work, professionalism, customer satisfaction, high performance, and mutual respect of others.
- Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
- Computer skills and dexterity for data entry and retrieval of patient information and ability to multitask.
- Knowledge of the welfare system.
- Understanding of financial statements to analyze trends and make recommendations and working knowledge of managing a budget.
- Ability to evaluate patient care data to ensure that the care is provided in accordance with clinical guidelines and organizational standards.
- Conflict resolution and problem-solving skills.
Responsibilities:
- Guides care management staff in all daily operational performance matters.
- Supervises the case managers and social workers responsible for patient care coordination, utilization review, intake and discharge planning.
- Utilizes all available financial and performance management information to align the staff with organizational goals.
- Serves as contact and resolution provider in customer service matters.
- Serves as patient experience educator for the case management staff to ensure delivery of outstanding customer service.
- Facilitates and contributes to leadership meetings within and outside of the case management department.
- Provides oversight of the department’s onboarding and annual staff performance evaluations.
- Adapts behavior to the specific patient population, including but not limited to: respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
- Performs other related duties as required.
