Responsibilities:

The Sydney Local Health District (SLHD) Community Transition Aged Care Program (TACP) is a goal-oriented program designed to assist eligible hospital inpatients to return to living at home safely, by providing low intensity therapies and assistance with activities of daily living for up to 12 weeks.
The Transition Care Case Manager ensures that local program therapies, services, documentation, communication, and administrative requirements meets the needs of the client, and relevant SLHD, NSW Ministry of Health, and Commonwealth Transition Care guidelines.
The position is a community role, covering the SLHD geographical area. TACP has its office base at Concord Hospital, but you may be asked to commence or finish work from other SLHD locations which include Royal Prince Alfred, Canterbury and Balmain Hospitals. There may be opportunities for mobile work arrangements and community based work-related activities will require use of your own car.
